I have just begun freelance graphic designing and am wondering if there is a template or standard phraseology I can/should use to sign-off and give my client full written copyright over the work I have done for them. Any tips or pointer?

It should have been in the contract that you were carrying out the work as “work for hire”.
“Work for hire” is a legal phrase that says that you were paid to do the work, but that the rights to the work reside with the person who done it. Your client can also register his work at http://www.provemycopyright.com
