i make designs using adobe photoshop and illustrator and now i need to design a large poster and get it printed at some printing office.. should i create the design with the actual poster dimensions? and what kind of file extention should i give to the printing office? thx

I was curious about web design. When designing a website, I have been told that most of the website is written using codes such as HTML, CSS, ect.
Is that how all websites or designs in general made? How is a website such as yahoo or facebook made?
My additional question is the actual design of the website. For example, facebook, how is that design created. The different images that are links. How are those created. Kind of hard to explain what I’m asking. But another example, the area at the top of this web page, there’s an area that…

So I designed my book cover in Adobe InDesign, and I have 9×12″ sheet of cardboard. My issue is that when I try to print it out, it is either in the wrong direction, wrong size, or both! How can I print it out in the correct size in the correct direction?
By the way, the cover from edge-to-edge is 7.5″ by 5.25″.
